29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  On or about September 20, 2018, at approximately 8:45 AM, an employee was working at Hewlett Parts and Accessories when he suffered an injury amputation resulting in hospitalization. Neither the amputation nor hospitalization were reported within 24 hours to the local Area Offices of the Occupational Safety and Health Administration. The Austin Area Office was notified on February 8, 2018, at approximately 3:30 PM, through an online submission from the employer.
