29 CFR 1910.147(c)(4)(ii): The procedures did not specifically outline the scope, purpose, authorization, rules and techniques to be utilized for the control of hazardous energy.     (a) Fabrication Shop: On February 4, 2019, the employer exposed employees to caught-in hazards in that a lockout or tagout device was not applied to the Bunting Company radius cutting band saw ("Saw #11") as authorized employees performed maintenance on the machine.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) Fabrication Shop: On or about February 4, 2019, and at times prior, the employer exposed employees to caught-in hazards in that the belt drive of the Bunting Company radius cutting band saw ("Saw #11") was unguarded as employees performed maintenance on the machine.
