OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to combustible dust deflagration and explosion hazards from several indoor dry-type dust collection systems handling various combustible dusts:    (a)   On or about March 6, 2019 a Donaldson Torit model #DFT2-16 [employer ID #68] cartridge media dust collector was located indoors within the Hawk #1 Department and collected a mixture of combustible dust containing phenolic resin, iron, barium sulfate, silicon carbide, carbon/graphite, magnesium oxide, epoxy powder coating, and other dusts from upstream grinding, sanding, grooving, and powder coating equipment/operations.  Employees were exposed to combustible dust explosion hazards (i.e. pressure wave, exiting flame front, projectile fragments) and deflagration propagation hazards (i.e. exiting and/or propagating flame front) from the indoor, unprotected collector.  The dust collector lacked means of explosion protection and deflagration propagation protection (isolation).      (b)   On or about March 6, 2019 a Donaldson Torit model #DFT2-16 [employer ID #159] cartridge media dust collector was located indoors within the Hawk #2 Department and collected a mixture of combustible dust containing phenolic resin, iron, barium sulfate, silicon carbide, carbon/graphite, magnesium oxide, and other dusts from upstream mixing, grinding, and pressing equipment/operations.  Employees were exposed to combustible dust explosion hazards (i.e. pressure wave, exiting flame front, projectile fragments) and deflagration propagation hazards (i.e. exiting and/or propagating flame front) from the indoor, unprotected collector.  The dust collector lacked means of explosion protection and deflagration propagation protection (isolation).
