This was most recently documented on or about February 13, 2019 at Bickford Senior Living, LLC located at 11308 Blondo Street in Omaha, Nebraska. A flexible cord was run through the ceiling in the Medication Room.

29 CFR 1910.1200(h): Employees were not provided effective information and training as specified in 29 CFR 1910.1200(h)(1) and (2) on hazardous chemicals in their work area:  Employees are not trained about the chemicals used throughout their work environment. This was most recently documented on or about February 13, 2019 at Bickford Senior Living, LLC located at 11308 Blondo Street in Omaha, Nebraska. Certified Nursing Staff (CNAs) routinely use QC 34 High Performance Ultra Concentrated Neutral Floor Cleaner, Sanitizing Wash N Walk, Home-Style Solid Detergent, Oasis 146 Multi-Quat Sanitizer, A-456 Disinfectant Cleaner, and QC 54 Peroxide Glass and Surface Cleaner on multiple surfaces at Bickford. Employees could not recall the name of the chemicals used, and many had not received training on Safety Data Sheets.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling.  Relocatable power taps (power strips) in the facility electrical room/storage room were not used according to the listing or labeling of the device. The power strips were used in sequence with an extension cord to supply auxiliary power for the communication system at Bickford. This was most recently documented on or about February 13, 2019 at Bickford Senior Living, LLC located at 11308 Blondo Street in Omaha, Nebraska. Maintenance personnel and housekeeping staff access the electrical room/storage room.   Bickford Senior Living, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 1910.303(b)(2), which is contained in OSHA inspection number 979554, Citation #1, Item #4, and was affirmed as final order on October 17, 2014.
