29 CFR 1910.30(a)(1): Before any employee is exposed to a fall hazard, the employer did not provide training for each employee who uses personal fall protection systems.    On or about February 6, 2019, an employee performing paint preparation activities on the roof of an electrical generator enclosure was exposed to fall hazards of approximately 12 feet and the employer did not ensure that the employee was trained to recognize fall hazards and methods to eliminate or control them.

29 CFR 1910.29(e)(1): The employer did not ensure that each cover for a hole in a walking-working surface is capable of supporting without failure, at least twice the maximum intended load that may be imposed on the cover at any one time:    On or about February 6, 2019, an employee performing paint preparation activities on the roof of an electrical generator enclosure fell approximately 12 feet after stepping on a covered roof cut-out and the employer did not ensure the cover could support twice the intended load.
