29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a.  On or about February 14, 2019, in the facility, employees were exposed to amputation hazards after the employer failed to establish and implement an adequate energy control program.  Employees used "e-stop" button(s) and "interlocked" equipment instead of ensuring an energy isolating device was affixed and that energized equipment was under the exclusive control of the exposed employee(s).

On or about February 14, 2019, in the facility, the employer had a written energy control program.  Employees were exposed to amputation hazards when the employer failed to conduct an annual periodic inspection of the energy control program.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation:    a.  On or about February 14, 2019,  on the Flanger machine, employees were exposed to amputation hazards when the machine did not have adequate guarding to protect employees from moving parts.    b.  On or about February 14, 2019, on the Curl & Bead machine, employees were exposed to amputation hazards when the machine did not have adequate guarding to protect employees from moving parts.      c.  On or about February 14, 2019, on the Pup Curl machine, employees were exposed to amputation hazards when the machined did not have adequate guarding to protect employees from moving parts.     d.  On or about February 14, 2019, on the Expander machine, employees were exposed to amputation hazards when the machine did not have adequate guarding to protect employees from moving parts.
