29 CFR 1926.1053(b)(7): Ladder(s) that were unsecured or not provided with slip-resistant feet were used on slippery surfaces:  On or about February 22, 2019, a Werner Aluminum Compact Attic Ladder with no slip-resistant and not securely bolted to the ceiling was placed on slippery Everbilt house wraps to gain access into the attic, exposing employees to fall from height hazards.

29 CFR 1926.1060(a)(1)(iii): The employer did not ensure that each employee using ladders and/or stairways had been trained by a competent person in the proper construction, use, placement, and care in handling the ladders and/or stairways:  On or about February 22, 2019 and times prior, the employer did not provide ladder safety training to employees installing attic ladders in a closet at a residential construction site, exposing them to fall from height hazards.
