29 CFR 1910.119(j)(4)(i): 29 CFR 1910.119(j)(4)(i): Inspections and tests were not performed on process equipment:  The employer is failing to protect employees from fire and explosion hazards while engaged in petroleum refining activities.  This was most recently documented on or about March 6, 2019 at the HollyFrontier El Dorado Refining LLC, located at 1401 Douglas Road in El Dorado, Kansas.  The employer did not ensure that a 90 degree elbow on the Low Pressure Gas Treater off-gas pipeline had received a mechanical integrity inspection for wall thickness.  The 90 degree elbow of the Low Pressure Gas Treater off-gas pipeline ruptured releasing a flammable gas that caused an explosion / fire.
