29 CFR 1910.147(c)(4): Procedures were not developed, documented, and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section.    On or about March 22, 2019 and at times prior thereto, the employer did not ensure that employees, engaging in unjamming/unclogging operations on elevator leg buckets, controlled potentially hazardous energy sources including, but not limited to utilizing documented procedures, ensuring locks are applied, and dissipation of all sources of energy. Employees were exposed to caught-in and amputation hazards.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees.    On or about March 22, 2019 and at times prior thereto, the employer permitted employees to work on energized equipment(clogged elevator leg) without ensuring energy isolating devices were utilized to control hazardous energy.  Employees were exposed to caught-in, struck-by, and amputation hazards.

29 CFR 1910.147(f)(3):  When servicing and/or maintenance was performed by a crew, craft, department or other group,  the employer did not ensure that employee(s) utilized a procedure which affords the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device.  On or about March 22, 2019 and at times prior thereto, the employer permitted multiple employees to work on energized equipment (clogged elevator leg) without ensuring that each employee was afforded the same level of protection equivalent to that provided by the implementation of a personal lockout or tagout device.  Employees were exposed to caught-in, struck-by, and amputation hazards.
