OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees exposed to caught-between hazards:     Fort Irwin Army Base Landfill:  On 03/26/2019, an employee suffered a fingertip amputation on his right pinky finger while operating the liftgate of a company vehicle. The employer did not adequately train employees to protect them from hazards while operating the equipment.
