29 CFR 1910.106(e)(2)(iv)(d): Flammable liquids were not drawn from or transferred into vessels, containers, or portable tanks within a building only through a closed piping system, from safety cans, by means of a device drawing through the top, or from a container or portable tanks by gravity through an approved self-closing valve: a) Hodge Foundry, Inc., Core room area, on or about April 11th, 2019 and times prior thereto: Employees were dispensing a category 4 flammable liquid (Techniset 1 6000 UNB pt1) in order to fill containers in the mixer cabinet of the core room. The dispensing valve for the flammable liquid was not self-closing.

Hodge Foundry, Inc., Core room area, on or about April 11th, 2019 and times prior thereto: Employees worked with and around corrosive chemicals including, but not limited to, Techniset 1 6000 UNB pt1. Employees did not have access within the immediate work area to a suitable eyewash/shower station.

Hodge Foundry, Inc., various production areas, on or about April 11th, 2019 and times prior thereto: Air sampling results for respirable crystalline silica indicated exposures above the permissible exposure limits for the following areas: Blaster-Clean area, Chipp-Clean area, Pit cleaning area, HRO-Core area, Hi Bay-Mold area, Mixer-Slinger area, Wheelabrator-Clean area and a laborer employee. The employer did not perform additional air sampling for these job operations/areas within 3 months from the previous sampling date.
