On or about 4/18/19 at the platform silo area located at 5601 Columbia Road, Grovetown, GA: Employees performing servicing and maintenance activities on the chain drive equipment and drum equipment were exposed to a fall hazard in excess of 100-feet, where the employer failed to install a guardrail system and or ensure the employees used a fall arrest system.
