29 CFR 1926.501(b)(4)(i):  Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:  (a)  Roof of Horse Stable:  On April 10, 2019 and times prior, employees were exposed to a fall of approximately 13 feet from the roof through holes approximately 38 inches wide by 78 inches in length while installing and removing metal roofing panels and skylights.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  (a)  Roof of Horse Stable:  On April 10, 2019  and times prior, employees were exposed to a fall from the roof of at least 13 feet to the ground below while installing and removing metal roofing panels and skylights on a roof with a slope of 6:12.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a)  Roof of Horse Stable:  On April 10, 2019 and times prior, employees were exposed to a fall of approximately 13 feet from the roof while installing and removing metal roofing panels and skylights.  The employer has not provided employees with training on recognizing fall hazards and the procedures required to be protected from the fall hazard.
