29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a)  On April 23, 2019, the employer did not ensure that energy control procedures were developed, documented and utilized to control the energy sources when employees were performing maintenance tasks such as, but not limited to, checking and changing the oil in the Bush Rotary Vacuum Pump on top of thermal press #2.   Employees were exposed to hazards such as, but not limited to burns while performing these tasks.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    a) On or about April 23, 2019, employees were exposed to injuries from moving parts and pressurized oil in that the thermal press #2 was not deenergized prior to performing servicing tasks such as, but not limited to, filling the Bush Vacuum Pump with oil.  The employer did not ensure that energy control application steps were implemented to control the machine's hazardous energy sources prior to the performance of servicing tasks to include:      �  Ensure that equipment was turned off and shut down.      �  Physically engage energy isolating devices to control the energy source(s).       �  Verify that isolation and de-energization of the machine or equipment had been accomplished.
