29 CFR 1926.600(a)(6)(i): For lines rated 50kV or below, a minimum of 10 feet was not maintained between the energized line and the equipment: a) Jobsite, One DRV Drive, Pittsburgh, PA 15221: On or about April 9, 2019, a minimum of 10 feet was not maintained between the work platform of a JLG brand articulating boom lift (model number 600AJ, serial number 0300160176) and overhead power lines. Two employees standing on the work platform of the lift were exposed to electric shock and electrocution hazards when the work platform made contact a 23 kV (phase to phase) overhead power line.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his environment to control or eliminate any hazards or other exposure to illness or injury: a) Jobsite, One DRV Drive, Pittsburgh, PA 15221: On or about April 9, 2019, the employer did not provide training to employees in the use of an articulating boom aerial lifts and the hazards associated with using articulating boom lifts in close proximity to overhead power lines. Two employees working in the basket of the lift were exposed to electric shock and electrocution hazards when the work platform of the lift made contact a 23 kV overhead power line.
