29 CFR 1910.28(b)(11)(ii): Each flight of stairs having at least 3 treads and at least 4 risers was not equipped with stair rail systems and handrails as required in Table D-2 of this standard:    a.  At the rear of the waste system, on or about 5/7/19:  A fixed stair having four risers was not provided with a stair rail system.    Paul Bunyan Products, Inc. was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 1316546, citation number 1, item number 1b and was affirmed as a final order on 7/6/18, with respect to a workplace located at 7101 NYS Route 281, Preble, NY 13141.

29 CFR 1910.265(c)(18)(i): Pulley(s), sprocket(s), sheave(s), drum(s) and block(s) of conveyor(s) were not arranged to prevent the possibility of injury due to hands or parts of clothing being caught between the belt(s) and pulley(s) or chain(s) and sprocket(s) or cable(s) and sheaves(s), drum(s) or block(s):    a.  In the waste system, specifically at conveyor 915, on or about 5/1/19:  Ingoing nip points at the conveyor belt and drums were not guarded.    b.  At the infeed to the stationary trim saw, on or about 5/7/19:  an ingoing nip point at the infeed chain to the stationary trim saw was not guarded.    Paul Bunyan Products, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1316546, citation number 1, item number 2 and was affirmed as a final order on 7/6/18 with respect to a workplace located at 7101 NYS Route 281, Preble, NY 13141.
