29 CFR 1910.28(b)(2)(i): The employer did not ensure employees in a hoist area were protected from falling 4 feet or more to a lower level:     Plant 2, mezzanine level: On or about April 30, 2019 and times prior thereto, the employer failed to ensure that employees were fall-protected, in that a link-chain was used in the hoist area, exposing employees to fall hazards of five (5) feet to the concrete floor below.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) Plant 2, mezzanine level: On or about April 30, 2019 and times prior thereto, the employer failed to ensure that a method of machine guarding was provided for employees taking ingredient samples from rotating screw feeders, thus exposing employees to amputation hazards.
