29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites:     A) On or about April 24, 2019, at the above referenced inspection site, employees were required to use electrical receptacles that were not protected by a GFCI, thereby exposing employees to an electrical shock/burn hazards.

29 CFR 1926.602(c)(1)(vi): Industrial truck(s) in use did not meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks:     A) On or about April 25, 2019, at the above referenced inspection site, employees were required to operate a Lull forklift in the performance of their duties. The employer failed to ensure the forklift had a functional back-up alarm or an operable seatbelt, thereby exposing employees to a struck by hazard and being thrown about inside the forklift.

29 CFR 1926.1052(b)(1): Foot traffic was not prohibited on stairways where the treads and/or landings of pan stairs were to be filled in with concrete or other material at a later date and stairs were not temporarily fitted with wood or other solid material at least to the top edge of each pan:     A) On or about August 28, 2019, at the above referenced inspection site, employees were required to traverse stairs in the performance of their duties. The employer failed to ensure the stairs pan treads were completely filled in with concrete or other material, thereby exposing employees to a fall hazard.
