29 CFR 1910.147(c)(6)(ii):  The periodic inspection certification of energy control procedures did not contain the required documentation on identification of the machine or equipment  inspection date, employees included, and the person performing the inspection:    The employer is failing ensure that employees are protected from the hazard of potential amputations, electrical injuries, and chemical burns from not documenting lockout reviews. This was most recently documented at 1307 South X Road, Hampton, Nebraska 68843 on May 2, 2019.   There was no documentation provided by the employer that showed the person performing the periodic inspection and the employees included in the inspection within the past year.  The last documented periodic inspection was September 21, 2016.      Abatement certification is required for this violation.  The abatement certification sheet is enclosed with the citations.
