29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:    (a) Pouch Machine 1 - Employees working on Pouch Machine 1 were exposed to stored energy when the interlock was bypassed, and no locks were applied, on or about May 24, 2019.  (b) Pouch Machine 2 - Employees working on Pouch Machine 2  were exposed to stored energy when the interlock was bypassed, and no locks were applied, on or about May 24, 2019.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  a) Pouch Machine 1 - Employees used a bypass key to disable interlocks when working on Pouch Machine 1 and were exposed to point of operation hazards, on or about May 24th, 2019.  b) Pouch Machine 2 - Employees used a bypass key to disable interlocks when working on Pouch Machine 2 and were exposed to point of operation hazards, on or about May 24th, 2019.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  a) Pouch Machine 1 - The employer did not perform an annual review of the lockout/tagout procedure for Pouch Machine 1, on or about May 24th, 2019.  b) Pouch Machine 2 - The employer did not perform an annual review of the lockout/tagout procedure for Pouch Machine 2, on or about May 24th, 2019.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
