29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary.  On or about June 19, 2019, employees were exposed to a fall of approximately 16 feet while installing siding materials on a new residential home located at 1457 Crescent, Sagimore Hills, Ohio 44067.  One employee was working from a pump jack scaffold and the employer did not ensure that the poles were secured at the base.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  On or about June 19, 2019, employees were exposed to a fall of approximately 16 feet while installing siding materials on a new residential home located at 1457 Crescent, Sagamore Hills, Ohio 44067.  One employee was working from the roof of the home and the employer did not ensure that he was using fall protection.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.  On or about June 19, 2019, employees were exposed to a fall of approximately 16 feet while installing siding materials on a new residential home located at 1457 Crescent, Sagimore Hills, Ohio 44067.  The employees were working from a pump jack scaffold and the employer did not ensure that they were using fall protection.  Tres Mulas Siding was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1), which was contained in OSHA inspection number 1201951, citation number 1, item number 1 and was affirmed as a final order on 1/26/2017, with respect to a workplace located at 2746 Hunters Ridge, Twinsburg, Ohio.
