29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) On or about June 20, 2019, in the production area, employees operating George Knight Presses were exposed to caught-in and burn hazards, in that the machine did not have sufficient guarding. (b) On or about June 20, 2019, in the production area, employees operating Synergy Speed Loopers were exposed to an amputation hazard, in that the machine did not have sufficient guarding.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. On or about June 11, 2019 and on or about June 14, 2019, the employer failed to report to OSHA an employee work related hospitalization/amputation within the required 24 hours. An employee crushed finger and amputation occurred on or about June 11, 2019 and employer notified OSHA on June 17, 2019.
