29 CFR 1926.1408(b)(1): When the employer was aware that the parts of the crane equipment, rigging and lifting accessories may get closer than the minimum approach distance of the power line permitted under Table A (see � 1926.1408) the employer did not conduct a planning meeting with the operator and the other workers who will be in the area of the equipment or load to review the location of the power line(s), and the steps that will be implemented to prevent encroachment/electrocution.     a) The employer did not conduct a crane planning meeting to ensure the crane will stay a minimum of 10 feet from the energized powerlines which were in the work area;  the crane was being operated within 5 feet of the overhead 240v energized powerline. The crane operator and other employees assisting the crane lift were exposed to electric shock hazard.
