On or about June 28, 2019, at the above addressed jobsite, an employee using a pneumatic nail gun did not wear safety glasses, thereby exposing the employee to the hazards associated with eye injuries.

On or about June 28, 2019, at the above addressed jobsite, an employee performing residential roofing operations thirteen (18') feet eleven (11") inches above the ground, was not protected from falls, thereby exposing the employee to fall hazards.    b.  On or about June 28, 2019, at the above addressed jobsite, employees performing residential roofing operations nine (9') feet three (3") inches above the ground, were not protected from falls, thereby exposing employees to fall hazards.
