29 CFR 1926.651(k)(2): Where the competent person found evidence of a situation that could result in possible cave-in, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions, exposed employee(s) was/were not removed from the hazardous area until the necessary precautions have been taken to ensure their safety.    (a) On or about July 12, 2019, the competent person allowed an employee to use a concrete cutting saw to cut an underground electrical wire without ensuring deenergization. This resulted in an electrical explosion and exposed employees to electrical shock and burn hazards.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).    (a) On or about July 12, 2019, an employee removing water and sewer lateral at seven feet in depth was not adequately protected from cave-in, thereby exposing the employee to struck-by / crushing hazards.    Dorner, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 1348352, citation number 1, item number 2a and was affirmed as a final order on November 21, 2018, with respect to a workplace located at Plymouth and Roosevelt St Oshkosh, WI 54901.
