A qualified person, meeting the definition of the standard, did not supervise the design, installation, and use of this anchor point as part of a complete personal fall protection system that maintained a safety factor of at least two, exposing employees who used the horizontal rectangular steel tube as an anchor point to the hazard of falling 4.75 feet from the tops of the mixers to the work platform and 8.5 feet from the tops of the mixers to the floor, on or about July 8, 2019.    (b) MST Room - In and above the ceiling next to the ventilation ductwork for Mixer 1, the employer fabricated and installed an anchor point for use as part of a personal fall arrest system by maintenance employees who worked from the tops of Mixer 1 and Mixer 2 to disassemble and reassemble ventilation piping that connected these mixers to ceiling ductwork.  A qualified person, meeting the definition of the standard, did not supervise the design, installation, and use of this anchor point as part of a complete personal fall protection system that maintained a safety factor of at least two, exposing employees who used the anchor point to the hazard of falling 4.75 feet from the tops of the mixers to the work platform and 8.5 feet from the tops of the mixers to the floor, on or about July 8, 2019.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
