29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:   Worksite located at 42 Bayview Trl.,  Lampe, Missouri- On or about July 9, 2019, employees were exposed to fall hazards while working on or near the third story deck which had the joists installed, but did not have the joists covered or decked over to prevent employees from falling between the joists. An employee fell to a lower level and sustained life-threatening injuries due to the fall.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye:   Worksite located at 42 Bayview Trl., Lampe, Missouri- On or about July 9, 2019, an employee had fallen from an elevated working level to a lower level in which he received multiple life-threatening injuries due to the fall. The employee was life-flighted to a hospital where he was admitted for treatment. The employer failed to contact the local area OSHA office to notify the office of the hospitalization.
