On or about July 17, 2019, roof area, employees were using a Gorilla ladder to install trim. The ladder was not extended 3 feet past the landing and was not tied off to prevent accidental displacement, exposing employees to fall hazards. b. On or about July 17, 2019, stairs, employees were using a Gorilla ladder to gain access and egress from the roof. The ladder was not extended 3 feet past the landing and was not tied off to prevent accidental displacement, exposing employees to fall hazards.
