29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a. On or about July 17, 2019, employees installing metal panels on an 8:12 pitch roof were not protected from falling 8.5 feet by the use of a fall protection system.

29 CFR 1926.1053(b)(4): Ladders shall be used only for the purpose for which they were designed.  a. On or about July 17, 2019, back side of building, employees were using one side of a 14-foot metal extension ladder which had a 3rd rung missing and there were no feet on the ladder, exposing employees to fall hazards.  This ladder was not being used as designed.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired:  a. On or about July 17, 2019, back side of building, employees were using one side of a 14-foot metal extension ladder which had a 3rd rung missing and there were no feet on the ladder, exposing employees to fall hazards.  The ladder was not marked as defective or taken out of service.
