29 CFR 1910.333(b)(2)(iv)(B): Before circuits or equipment was worked as deenergized, a qualified person did not use test equipment to test the circuit elements and electrical parts of equipment to which employees would be exposed and did not verify the conductor's polarity.  Worksite: On or about July 22, 2019, the lead lineman (Qualified Person) did not use test equipment to test the circuit polarity. This condition exposed employees to electrical shock and arc flash hazards.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not using electrical protective equipment that was appropriate for the specific parts of the body to be protected and for the work to be performed:  Worksite: On or about July 22, 2019, the lead lineman (competent person) did not asses the workplace for adequate Personal Protective Equipment (PPE) while employees were working with live electricity. This condition exposed employees to electrical shock and arc flash hazards.

Worksite: On or about July 22, 2019, the lead lineman (Qualified Person) did not use or neither provided his assistant with a face shield. This condition exposed employees to electrical shock and arc flash hazards.
