29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  On or about July 17, 2019 the employee cleared a jam on the transfer belt without de-energizing the transfer belt and had the tip of his finger amputated.

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s):  On or about July 17, 2019 the employee cleared a jam on the transfer belt without de-energizing the transfer belt and had the tip of his finger amputated.
