29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  At a jobsite located at 4121 Northeast Davidson Road, Kansas City, Missouri 64116, employees were exposed to serious fall hazards. Employees conducting a powered paint spraying application on the roof of the self-storage building was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. Employees were exposed to a fall hazard of between 8 feet 6 inches to 10 feet 6 inches to the ground below.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  At a jobsite located at 4121 Northeast Davidson Road, Kansas City, Missouri 64116, employees engaged in roofing work on a low sloped roof were exposed to serious fall hazards. A portable ladder which was being used to access an upper landing surface and the side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access. Employees were exposed to an 8 foot 6 inch fall hazard to the ground below.
