29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent) was not completed in the detail as required by the regulation:    Graymills Corporation, Broadview, IL - On or about August 2, 2019, the employer failed to record the following workplace injuries in the detail required by the regulation on the OSHA 300 Log for calendar year 2019.    a. On or about July 9, 2019, a recordable injury occurred when an lead man in the machine shop sustained a right index finger laceration while putting away a 2.5 inch round metal bar about 6 feet long onto a rack.  The bar slipped and pinched the employee's finger causing the laceration, which resulted in the employee requiring stiches. The information in Column F was not recorded in the detail required, as it did not describe the object that directly injured the employee.    b. On or about August 26, 2019, a recordable injury occurred when a lead employee in the fabrication department sustained a laceration on the left index finger while unjamming a piece of sheet metal that was stuck on the laser bed, resulting in the employee requiring stiches.  The information in Column F was not recorded in the detail required, as it did not describe the object that directly injured the employee.    c. On or about September 20, 2019, a recordable injury occurred when a painter employee received a contusion to the left wrist when a tank's lift straps got tangles and tank fell, resulting in 3 days of lost time. The information in Column F was not recorded in the detail required, as it did not describe the object that directly injured the employee.
