29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):    (a)  BIN 12, NEAR ENTRY DOOR - Employees operated the breakers inside of the orange electrical cabinet on the outside of Bin 12, and the employees were exposed to the live unguarded 3 phase 240 volt electrical conductors entering and exiting the three magnetic motor starters at the bottom of the cabinet, on or about August 5, 2019.    (b)  CONTROL ROOM, MAIN CONTROL PANEL - Employees reset the motor control starters in the control panel for the bin auger and fan systems located in the control room, and the employees were exposed to the live unguarded 3 phase 240 volt electrical conductors entering and exiting the 18 magnetic motor starters located throughout the cabinet, on or about August 5, 2019.     Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings were not protected from abrasions:    (a)  EXTERIOR BIN 72, NEAR ENTRY DOOR - Employers used a sweep auger to empty the bin and had plugged the sweep auger into a temporarily wired plug and cord assembly.  The plug and cord assembly had been wired so the conductors passed through an opening in the bottom of the disconnect switch, and there was nothing to protect the uninsulated conductors from abrasion, on or about July 4, 2019.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:  (a)  EXTERIOR BIN 72, NEAR ENTRY DOOR - Employers used a sweep auger to empty the bin and had plugged the sweep auger into a temporarily wired plug and cord assembly.  The plug and cord assembly had been wired so that there was nothing to prevent tension being applied to the conductors as they exited the disconnect, on or about July 4, 2019.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
