29 CFR 1910.36(d)(3): An exit route door may be locked from the inside only in mental, penal, or correctional facilities and then only if supervisory personnel are continuously on duty and the employer has a plan to remove occupants from the facility during an emergency.  Buskin River Road, Administrative Building, Southern Emergency Exit Door: on or about 30 July 2019, an emergency exit door was locked from the inside by a deadbolt style lock. This condition exposed employees to delays in exiting the building and hazards associated with smoke inhalation and burns from fire.

29 CFR 1910.36(h)(3): Outdoor exit route(s) were not reasonably straight and did not have smooth, solid, substantially level walkways:  Buskin River Road, Administrative Building, southern emergency exit: on or about 30 July 2019, overgrown brush measuring approximately 48 inches in height and 36 inches width was present on the exterior pathway of an emergency exit route. This condition exposed employees to hazards associated with trips or falls due to walkways not being smooth and substantially level.

29 CFR 1910.37(a)(3): Exit routes were not kept free and unobstructed. Materials or equipment were placed, either permanently or temporarily, within the exit route:  On or about 30 July 2019, exit routes were blocked in two instances obstructing the path of travel.  A) Buskin River Road, Administrative Building, southern facing emergency exit adjacent to Bill's office: a coat rack and storage container were placed in the doorway blocking the path of travel.   B) Buskin River Road, Maintenance Shop: a portable step ladder was placed in the doorway leading from the maintenance shop through the boat shop to an emergency exit blocking the path of travel.   These conditions exposed employees to delays in exiting the building and hazards associated with smoke inhalation and burns from fire.
