29 CFR 1926.21(b)(2):   The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  a)  37 Vreeland Road, Florham Park, NJ and at the Corporate Offices at 411 Hackensack Avenue, Hackensack, NJ.  Workers were exposed to injury from, but not limited to building demolition site hazards and material handling hazards involving a skid steer grapple bucket loader, as the employer had not provided training and instruction on recognizing and avoiding unsafe conditions on a large building demolition site.                                                          Violation observed on or about 07/30/2019.  b)  37 Vreeland Road, Florham Park, NJ at the Corporate Offices at 411 Hackensack Avenue, Hackensack, NJ.  Workers were exposed injury from, but not limited to building demolition site hazards and struck-by hazards from materials above, as the employer had not provided training and instruction regarding recognizing and avoiding unsafe conditions on a large building demolition site.                                                             Violation observed on or about 08/06/2019.

Workers were exposed to struck-by injuries from the building's materials on this demolition site as they were not wearing hard hats while inside the building. Workers were exposed to injury from, but not limited to building collapse during demolition as the employer had not provided for an engineering survey to be performed by a competent person prior to the start of demolition.                                                    Violation observed on or about 07/30/2019 and 08/06/2019.
