29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about July 12, 2019, employees performing residential construction were not provided fall protection and exposed to unprotected fall hazards of approximately 9 feet to the ground below.  An employee fell and sustained broken bones.

29 CFR 1904.39(a)(2):  The employer did not report within twenty-four (24) hours, the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident.  At the establishment, a work-related injury occurred and the employer failed to report to OSHA the inpatient hospitalization prior to the elapse of the 24 hour reporting period of the incident and thereafter.
