29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:  On or about August 9, 2019, an employee using an electric drill was exposed to electric shock hazards because the insulation of the extension cord used to power the drill was damaged near the female plug end.

29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupers for personal protection:  On or about August 9, 2019, employees using electric power tools were exposed to electric shock hazards and a ground-fault circuit interrupter was not used.
