29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:  a) On or about 8/5/19 at a roof project located at 381 Huntington Avenue, Buffalo, New York; employees performing roofing activities from a ladder jack scaffold approximately 18 feet high were not protected from falling by a personal fall arrest system.  NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) On or about 8/5/19 at a roof project located at 381 Huntington Avenue, Buffalo, New York; employees were placing new sheathing and shingles on the roof of a two story house. The roof had a 8 in 12 pitch and an eave height of approximately 19 feet. There was no fall protection in use.  NO ABATEMENT CERTIFICATION REQUIRED
