29 CFR 1910.303(b)(7)(iv): Electric equipment had damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating: On or about August 20, 2019, on the west side of the Service Department where employees were exposed to electrical hazards while using and working near an electrical receptacle box that was broken without ensuring that the damaged part did not affect the safe operation of the equipment.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: On or about August 20, 2019, on the east side of the Service Department where employees were using a trolley to move tubing spools that had an emergency stop button was not connected to the trolley so that strain relief was provided to prevent pull from being applied to terminal screws.

29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical: On or about August 20, 2019, in the Service Department where employees were transferring a corrosion inhibitor into containers without marking the containers with the chemicals name or general hazard information.
