Workers were tearing off the old shingles and installing new shingles. They were working approximately three stories high at an apartment complex. Workers were exposed to a fall hazard.

29 CFR 1926.602(c)(1)(ii): Modifications or additions affecting the safe operation of the equipment were  made without the manufacturer's written approval:  On or about August 22, 2019, the employer failed to get to manufacture approval prior to attaching the job-made wooden trash box onto the JLG telehandler. The employer was hauling old shingles from the roof to the ground in a job-made wooden trash box that was not attached properly.  Workers expose to a struck-by hazard.
