29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):     On or about August 20, 2019, employees performed a hot tap procedure to vent a pressurized well head at the Lloyd Bigelow #2 well site. The employer did not ensure that a written hazard assessment was developed and established that included the need of personal protective equipment such as but not limited to face shields.

The employer did not ensure that employees utilized personal protective equipment such as but not limited to face shields.
