29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.   On or about 8/08/2019, jobsite located at 3057 Coral Springs Drive, Coral Springs, FL 33065, employees were using a ladder that was not secured at its top to a rigid support, upper level or landing exposing employees to a fall hazard.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.  On or about 8/08/2019, jobsite located at 3057 Coral Springs Drive, Coral Springs, FL 33065, employees were using a 32-feet fiberglass extension ladder which was defective to access the second floor, exposing the employees to a fall hazard.
