The employer must ensure: Walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice. a) On or about August 20, 2019 an employee received a partial amputation when their foot was caught in a pinch point created by the wedge hole of the trunnion and the degas vessel platform when raising the platform during a degas vessel rotation.

29 CFR 1910.22(d)(1): Inspection, maintenance, and repair. The employer must ensure: Walking-working surfaces are inspected, regularly and as necessary, and maintained in a safe condition; a) On or about August 20, 2019 the employer did not ensure that the degas vessel platform was inspected and maintained in a safe condition in that during the degas vessel rotation there is a pinch point created by the wedge hole of the trunnion and the platform when the platform is raised.
