29 CFR 1926.1053(b)(16): Portable ladder(s) with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately:    45 Honeybee Road, Dracut, MA    On or about August 30, 2019, employees were exposed to fall hazards from ladders that had bent rungs that could give way when stepped on, causing the employee to fall.     a) An aluminum ladder that extended to the roof on the left hand side of the house entrance was found to have the third fro the top run to be substantially deformed downwards.     b) An aluminum ladder that extended to the lower roof of the right hand side of the house was found to have the top three rungs to be substantially deformed downwards, with the middle rung appearing to be severed in half.    Trancis Fast Construction Corporation was previously cited for a violation of an equivalent Occupational Safety and Health Standard, 29 CFR 1926.1051(a), which was contained in OSHA inspection number 1203863, citation number 1, item number 2 and was affirmed as a final order on June 16, 2016, with respect to a workplace located at 357 Central Street, Manchester NH, 03101.

Employees could fall from the ladder or roof when attempting to access either the ladder or roof.     a) An aluminum ladder that extended to the roof on the left hand side of the house entrance was found to have approximately 1 rung raised above the roof surface.     b) An aluminum ladder that extended to the lower roof of the right hand side of the house was found to be extended to meet the peak of the subroof, and had 1 run raised above the roof surface.
