29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    On or about 09/10/2019 at 8855 Collins Ave. Miami, FL, employees were exposed to a fall hazard of 10 feet to ground level while using a ladder that did not extend 3 feet above the landing surface of a temporary working platform.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access:    On or about 08/23/2019, at 8855 Collins Ave. Miami, FL, an employee was exposed to a 10 feet fall hazard while using the cross braces of a temporary working platform to ascend and descend its upper level.
