29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows: a) On September 9, 2019, located north side of the building, an employee was exposed to a fall hazard while welding bar joists utilizing a mobile scaffold which was not fully planked in that scaffold was only planked one plank wide leaving an approximate 30 inch section of the work platform unplanked. Fall height measured to be 11 feet 8 inches. b) On September 9, 2019, located south side of the building, an employee was exposed to a fall hazard while welding bar joists utilizing a scaffold which was not fully planked in that scaffold was only planked one plank wide leaving an approximate 30 inch section of the work platform unplanked. Fall height measured to be 13 feet 7 inches.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components shall be inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity. a) On September 9, 2019, located on the North side of a commercial structure, an employee were exposed to a fall hazard working from a scaffold system as there were no inspections performed by a competent person prior to the work shift to ensure employees were protected from falling by installing a personal fall arrest system, or ensuring the scaffolding was fully planked. Fall height measured to be 11 feet 8 inches. b) On September 9, 2019, located on the South side of a commercial structure, an employee were exposed to a fall hazard working from a scaffold system as there were no inspections performed by a competent person prior to the work shift to ensure employees were protected from falling by installing a personal fall arrest system, or ensuring the scaffolding was fully planked. Fall height measured to be 13 feet 7 Inches.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers. a) On September 9, 2019, located on the North side of the commercial structure an employee was not protected from falling while placing bar joist from a tubular welded frame scaffold system. Potential fall hazard measured to be 11 feet 8 inches. b) On September 9, 2019, located on the South side of the commercial structure an employee was not protected from falling while placing bar joist from a tubular welded frame scaffold system. Potential fall hazard measured to be 13 feet 7 inches.
