29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees: a) Job site, Undetermined address near Rochester Road, Cranberry, PA: On or about September 5, 2019, during excavation work, the employer did not ensure that a safe means of egress was provide in an excavation over 4 feet deep in that a ladder placed in the excavation was damaged, leaving only a rocky excavation wall which could slide as you climbed it, for egress.
