29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about 9/19/19, during residential roofing activities located at 1466 Jude Avenue in Streetsboro Ohio, the employer failed to ensure that employees were provided and utilized fall protection while installing roofing material. The roof height varied in height between nine and one-half feet (9.5') to eleven feet (11'). The slope of the roof was 8/12.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about 9/19/19, during residential roofing activities located at 1466 Jude Avenue in Streetsboro Ohio, the employer failed to ensure that the unknown make and model number extension ladder was extended at least three feet (3') above the roof, or landing surface, when used to access the roof. The height of the roof where the ladder was setup was nine and one-half feet (9.5'). The ladder was about two feet (2') above the roof, or landing surface.
