29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  The employer is failing to protect employees from fall hazards while performing residential roofing operations.  This was most recently observed at 8616 Hickory St, Omaha, Nebraska on October 9, 2019 when 5 workers were on the roof of a residential structure with an eave height over 8' and a pitch of 10 in 12 and the workers were not using fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet above the upper landing surface being accessed:  The employer is failing to protect employees from fall hazards while using ladders.  This was most recently observed at 8616 Hickory St, Omaha, Nebraska on October 9, 2019 when 5 workers were using a ladder to access to the roof of a residential structure and the ladder did not extend at least 3 feet past the upper landing surface.  The roof eave height was over 8 feet above the ground.
